The questionnaire
TRAQ 6.0 (2021): 20 skill items in four domains, plus two questions on importance and confidence. Written at about a 6th-grade reading level and completed in 3–5 minutes.
Response scale
Youth rate each skill on five levels. Each level lines up with a stage in the Transtheoretical Model of behavior change, which tells the care team what kind of support will help most.
| Score | Response | Stage of change | What it means |
|---|---|---|---|
| 1 | No, I do not know how | Pre-contemplation | Not interested in this yet, or not thinking about it. |
| 2 | No, but I want to learn | Contemplation | Open to trying, but hasn't tried on their own yet. |
| 3 | No, but I am learning to do this | Initiation | Early efforts; not yet comfortable doing it. |
| 4 | Yes, I have started doing this | Action | Doing it, but not yet consistently. |
| 5 | Yes, I always do this when I need to | Mastery | Does it reliably and independently. |
The 20 items
Internal consistency (Cronbach's α) is shown for each domain. Overall scale α = 0.930 (standardized 0.933).
Importance & confidence
How important is it to you to manage your own health care?
1 = Not at all important · 5 = Very important
How confident are you in your ability to manage your own health care?
1 = Not at all confident · 5 = Very confident
These two items are scored separately and are not part of the total. A low score on either is a cue to explore why, using the approaches in How to Use.
Scoring
Score each item 1–5
1 = "No, I do not know how" … 5 = "Yes, I always do this when I need to."
Average each domain
Take the mean of the items answered in each of the four domains.
Average the total
Take the mean across all items answered. Look at the pattern, not only the total.
Ready to transfer
When youth consistently average 4.0 or higher in every domain, they have the core skills to manage their own care and are ready to move to an adult provider.
